Some family doctors receive the basic obstetric training, so they can do the delivery for the low risk patients(normal pregnancy). These family doctors pay certain amount of insurance every year to allow them to "make this money". If you choose the family doctor to do the delivery, then your care will be under the family doctor. The OB on call will not assess you unless you need cesarean section or other interventions which are out of the family doctor's practice scope. In this case, the family doctor will be your primary care doctor, she/he has to come to delivery your baby because this is her/his responsibility. They get money from the government and you do not need to pay them. In other worlds, the on call doctors' are obstetricians and their skills and knowledge are much better than family doctors. In Toronto, most of obstetricians do not charge for any extra delivery fee, except the Chinese doctors who work in the North York General Hospital.
